Need to book an appointment? Now, not only can you book your appointments in person or over the telephone, you can now book them online as well! In addition to booking appointments with your current doctors, you can now use our online booking tool to find other doctors and specialists in your area. Once you enter your procedure and location, the system will list all of the doctors that perform that procedure, sorted by region and specialty. It also lets you know where the doctor is located, and what days/hours they are open.

Requesting an appointment is just as easy. Go to the appointment page, and give them your best possible day(s) and time(s). The system will schedule a new appointment. From these pages you can also view upcoming appointments, or reschedule or cancel existing appointments.

When you arrive at your appointment, please be sure to bring: =

• A valid photo ID (such as driver’s license)
• Your referral number
• If you have insurance, your insurance card
• If you are uninsured or underinsured, your last paycheck stubs
• The name of your doctor or referring physician
