Claims and Explanation of Benefits (EOB)
Each time a claim is submitted, an Explanation of Benefits (EOB) is generated.
An EOB is not a bill.
It simply explains how your benefits were applied to that particular claim. The EOB should contain that date of service, the amount billed, the amount paid by your healthcare provider, and any outstanding balances you are responsible for, such as co-pays, deductibles, and products and services not covered by your plan. The EOB also lets you know how much has been credited towards your annual deductible.
