ID Cards (Get Your Health Plan ID Card)

Every time that you visit, you should bring your health care ID card. This card contains important information, such as:
  • The name and contact information of your healthcare provider
  • Your member identification number.
  • Your member group number, which identifies the your plan benefits
  • Your RX group number, which is used to process your prescriptions
  • The date that your card was issued
  • The name of your health care provider network
  • Your co-pay amount for both office visits and emergency room visits

If you did not receive an ID card the first time you visited that facility or hospital, then you can order one online by going to Order an ID card. The suitcase icon on your card indicates that your plan participates in a program that receives certain levels of health-care benefits wherever you live or travel, across the country and worldwide.
